Smart Locations Site Readiness form update
To help deliver the most accurate pricing for Smart Locations connections, we use the Smart Locations Site Readiness form. Based on your feedback, we’re simplifying the form to remove fields already captured in the Chorus Portal.
What’s changing?
- The number of mandatory fields has been reduced from 19 to 11.
- The form now features a refreshed, modern simple design.
- This change applies to direct-to-Chorus Portal ordering (NBAP) only and does not impact the Customer Fibre Delivery (CFD) process.
When is this happening?
We’re giving you 14 days’ notice to ensure a smooth transition, which includes 1 week for you to respond with any feedback or concerns.
- Notice date: 02 July 2025
- Feedback deadline: 9 July 2025
- Implementation date: 16 July 2025
Fields removed
- Service Provider order reference
- Smart Location Address TLC
- Has permission been gained from the Land Owner if on private property for Chorus to run fibre to the Smart Location? (Y/N)
- Consent Permit signed and returned to Chorus? (Y/N)
- Existing Chorus service TLC or SAM ID (Address)
- Specific accreditation or induction instructions for the site?
- Health & Safety requirements or information? (e.g. hardhats, PPE etc)
What this means for you
No action is required unless you anticipate a significant impact on your processes. If you do, please reach out to us before 9 July 2025.
From 16 July 2025 onwards, please use the updated form available on the Chorus Service Provider website. Alternatively, our Provisioning team will continue to provide the form with each order or application.
